She died on leukemia so radiation poisoning is to blame. But especially one accident - a sealed capsule of Polonium exploded - probably it produced so many alphas that it was hot and it created a high pressure.
So, Irene died mainly of Polonium poisoning like a Russian guy who was poisoned by a Soviet spy.
